You may file a “Stage I” appeal of your disqualification from an examination or failure of an examination. You must provide supporting evidence for your reason(s) for appealing either disqualification or failure. If you provide such supporting evidence, we will re-evaluate your disqualification and/or test results.
In your appeal letter, you must provide all information necessary to support your basis for an appeal. Failure to present such information will result in no action being taken on your Stage I appeal .
Your Stage I appeal must be in writing postmarked no later than two weeks from the date of the letter notifying you of your disqualification and/or test result. Send your written appeal to:
